Indiana Licensed Telehealth Nurse

Job Summary

To manage symptom-based encounters with patients, the full-time Indiana Licensed Telehealth Nurse will triage calls and video consultations, utilizing evidence-based protocols to provide care recommendations and promote self-care, with the opportunity to work remotely after training.

Key responsibilities

• Triages patient calls by assessing symptoms and determining appropriate care actions
• Provides health education and navigates patients through the healthcare system
• Completes Transitional Care Management encounters for post-hospital patients

Required qualifications

• Current, active RN license with a compact license or application for Nurse Licensure Compact
• Graduate of a School of Nursing, BSN preferred
• Minimum of five years of experience as a registered nurse, with preferred experience in emergency department or telephone triage
• Proficient in using electronic medical records and computer applications
• Strong assessment skills and excellent clinical judgment
