Job Summary

Providing telehealth services to patients across Indiana, the part-time Indiana Licensed Telehealth Provider will evaluate and treat patients through personalized hormone replacement therapy, medical weight management, and preventive healthcare in a fully remote setting.

Key responsibilities

• Evaluate and treat patients through evidence-based telemedicine practices
• Build long-term patient relationships while delivering compassionate care
• Collaborate in a telemedicine environment to optimize patient health and wellness

Required qualifications

• Active Indiana Family Nurse Practitioner (FNP) license or Physician Assistant license
• Current Indiana DEA and CSR registrations at the time of application
• Current National Provider Identifier (NPI) and applicable board certification
• Strong clinical assessment and medical decision-making skills
• No current or pending malpractice claims or disciplinary actions
