Job Summary

A company is looking for a Telehealth Triage Nurse - RN.

Key Responsibilities

• Triages patient calls and manages symptom-based encounters using evidence-based protocols
• Provides health education, medication assistance, and recommendations for next steps in care
• Completes Transitional Care Management encounters for post-hospital patients

Required Qualifications, Training, and Education

• Graduate of a School of Nursing; BSN preferred
• Current, active RN license with a compact license or application for it
• Minimum of five years' experience as a registered nurse
• Previous emergency department or telephone triage experience is desired
• Experience with electronic medical records and computer systems
