Job Summary
To support patient care through effective symptom assessment, the full-time Indiana Licensed Telehealth Triage Nurse will manage phone and video encounters, utilizing evidence-based protocols while working primarily on Thursdays, alternating days, and every other weekend from 8 PM to 8 AM, with the potential for remote work after training.
Key responsibilities
- Accept and triage patient calls, determining appropriate care and next steps based on clinical protocols
- Provide health education, medication assistance, and recommendations for self-care and follow-up appointments
- Conduct Transitional Care Management encounters for post-hospital patients, ensuring effective communication and care navigation
Required qualifications
- Graduate of a School of Nursing; BSN preferred
- Current, active RN license with a compact license or application for the Nurse Licensure Compact
- Minimum of five years of experience as a registered nurse, with preference for emergency department or telephone triage experience
- Proficiency in using electronic medical records and basic computer skills
- Strong critical thinking, assessment skills, and the ability to communicate effectively in high-stress situations
