Indiana Licensed Telehealth Triage Nurse

Job Summary

To support patient care through effective symptom assessment, the full-time Indiana Licensed Telehealth Triage Nurse will manage phone and video encounters, utilizing evidence-based protocols while working primarily on Thursdays, alternating days, and every other weekend from 8 PM to 8 AM, with the potential for remote work after training.

Key responsibilities

• Accept and triage patient calls, determining appropriate care and next steps based on clinical protocols
• Provide health education, medication assistance, and recommendations for self-care and follow-up appointments
• Conduct Transitional Care Management encounters for post-hospital patients, ensuring effective communication and care navigation

Required qualifications

• Graduate of a School of Nursing; BSN preferred
• Current, active RN license with a compact license or application for the Nurse Licensure Compact
• Minimum of five years of experience as a registered nurse, with preference for emergency department or telephone triage experience
• Proficiency in using electronic medical records and basic computer skills
• Strong critical thinking, assessment skills, and the ability to communicate effectively in high-stress situations
