Intake Admission Specialist

Job Summary

To support patient transitions to home care services, the full-time Intake Admission Specialist will manage referral and intake operations, communicate with various stakeholders, and ensure compliance with regulations while working remotely in Michigan.

Key responsibilities

• Processes daily patient referrals and intake operations according to established policies
• Communicates with service providers to coordinate care and follows up on patient satisfaction regarding services
• Maintains knowledge of community resources and assists referral sources in accessing necessary services

Required qualifications

• Associate's degree and/or two years of experience in the healthcare field, including one year in home care
• Familiarity with medical terminology and knowledge of governmental regulations and private payer practices
• Strong negotiation and public relations skills with the ability to create positive impressions
• Ability to multi-task, prioritize, and track data effectively
• Personable and courteous with a focus on building working relationships
