Intake Admission Specialist

Job Summary

To support patient transitions to home care services, the full-time Intake Admission Specialist will manage referral processes, maintain communication with various service providers, and ensure compliance with healthcare regulations, primarily working remotely with some in-office responsibilities in the Detroit/Troy area.

Key responsibilities

• Processes daily patient referrals and intake operations, ensuring adherence to policies
• Communicates with service providers to coordinate patient care and follows up on Start of Care services
• Maintains knowledge of community resources and assists referral sources in accessing necessary services

Required qualifications

• Associates degree and/or two years of experience in healthcare, with one year in home care
• Familiarity with medical terminology and knowledge of governmental regulations
• Ability to create positive impressions and communicate effectively with diverse individuals
• Strong negotiation and public relations skills
• Detail-oriented with the ability to multi-task and prioritize effectively
