Intake Admission Specialist

Job Summary

To support patient transitions to home care services, the full-time Intake Admission Specialist will manage referral and intake operations, communicate with referral sources and staff, and maintain relationships with community resources, primarily working remotely with some in-office training required in the Detroit/Troy area.

Key Responsibilities

• Processes daily patient referrals and intake operations in accordance with policies
• Maintains knowledge of contractual relationships to ensure proper patient admissions
• Coordinates care by communicating with multiple service providers and following up on patient satisfaction

Required Qualifications

• Associates degree and/or two years of experience in the healthcare field, including one year in home care
• Familiarity with medical terminology and knowledge of governmental regulations
• Proven ability to create positive impressions and communicate effectively with diverse individuals
• Experience in marketing and public relations within a healthcare context
• Strong data tracking and input skills, with attention to detail
