Job Summary

A company is looking for an Intake Admissions Specialist to facilitate patient transitions to home care services.

Key Responsibilities

• Process daily patient referral and intake operations
• Coordinate care with service providers and obtain physician orders as necessary
• Follow up with patients regarding Start of Care and service satisfaction

Required Qualifications, Training, and Education

• Associates degree or equivalent healthcare/home care experience required
• Two years of healthcare experience with one year in home care preferred
• Experience in Medical Terminology preferred
• Knowledge of government regulations and private payer practices
• Capable of meeting all physical demands of the role
