Intake Admissions Specialist

Job Summary

To support patient transitions to home care services, the full-time Intake Admissions Specialist will manage patient referrals, communicate with healthcare providers, and ensure accurate data entry while working remotely.

Key responsibilities

• Process daily patient referral and intake operations
• Coordinate care by communicating with service providers and obtaining necessary orders from physicians
• Follow up with patients regarding Start of Care and service satisfaction

Required qualifications

• Associates degree or equivalent healthcare/home care experience required
• Two years of healthcare experience, with one year in home care preferred
• Experience in Medical Terminology is preferred
• Knowledge of government regulations and private payer practices
• Ability to meet all physical demands of the role
