Intake Admissions Specialist
Job Summary
To support patient transitions to home care services, the full-time Intake Admissions Specialist will manage patient referrals, communicate with healthcare providers, and ensure accurate data entry while working remotely from 8:30 AM to 5:30 PM, Monday through Friday, with weekend on-call and holiday rotation required.
Key responsibilities
- Process daily patient referral and intake operations efficiently
- Coordinate care with service providers and obtain necessary orders from physicians
- Follow up with patients regarding Start of Care (SOC) and service satisfaction
Required qualifications
- Associate's degree or equivalent healthcare/home care experience required
- Two years of healthcare experience, with one year in home care preferred
- Experience in medical terminology is preferred
- Knowledge of government regulations and private payer practices
- Ability to meet all physical demands of the role
