Job Summary
To support patient onboarding in the GUIDE program, the full-time Intake and Engagement Specialist will conduct onboarding calls, verify patient information, and coordinate with clinical teams, all while working remotely.
Key Responsibilities
- Conduct onboarding calls with patients and caregivers to guide them through the intake process
- Collaborate with clinical teams and the Partner Relationship Team to ensure a smooth transition from intake to care delivery
- Maintain accurate patient records and documentation, ensuring all onboarding steps are fulfilled
Required Qualifications
- 1-3 years of experience in healthcare, patient intake, care coordination, or similar roles
- Associate's degree or 5+ years of relevant experience
- Experience in patient-facing roles, such as intake coordination or customer support
- Familiarity with digital platforms for tracking and scheduling, with experience in HubSpot and/or Athena being beneficial
- Strong organizational skills and the ability to manage multiple tasks in a fast-paced environment
