Job Summary

A company is looking for an Integrated Duals Program Coordinator II.

Key Responsibilities
  • Support care coordination and engagement activities, ensuring timely and member-centered services
  • Act as the first point of contact for complex member and provider needs, coordinating responses and mentoring junior staff
  • Maintain accurate documentation and monitor service delivery to ensure compliance and appropriateness of care
Required Qualifications, Training, and Education
  • High School diploma or GED required; Bachelor's degree in a social science or healthcare-related field preferred
  • Three years of experience in healthcare required; four years preferred
  • Intermediate proficiency in Microsoft Office applications
  • Knowledge of local, state, and federal healthcare laws and regulations
  • Strong understanding of cultural diversity and community support resources
