Job Summary
Supporting care coordination and engagement activities, the full-time Integrated Duals Program Coordinator II will manage complex escalations, mentor staff, and ensure timely, member-centered services in a remote environment.
Key responsibilities:
- Act as the first point of contact for complex member and provider escalations, coordinating timely responses
- Mentor and support Integrated Duals Program Coordinator I staff, ensuring consistency in documentation and workflow
- Maintain accurate documentation and collaborate with internal clinical teams to ensure continuity of care
Required qualifications:
- High School diploma or GED required; Bachelor's degree in a social science or healthcare-related field preferred
- Three years of experience in healthcare required; four years preferred
- Intermediate proficiency with Microsoft Office, including Outlook, Word, and Excel
- Knowledge of local, state, and federal healthcare laws and regulations
- Strong understanding of community and state support resources
