Job Summary

To support our care teams, the full-time Integrated Kidney Care Advocate I will be responsible for identifying and validating hospital admissions for Transition of Care support, providing administrative assistance, and managing communication tasks, all while working remotely from the Central, Mountain, or Eastern Time Zone.

Key responsibilities

• Confirming the status of hospital admissions and conducting regular check-in calls to verify admission status
• Providing administrative support by obtaining and uploading medical records and addressing ad hoc requests
• Monitoring and processing incoming and outgoing faxes while maintaining the fax log

Required qualifications

• High School Diploma required
• 2 years of clinical administrative experience preferred
• Intermediate proficiency in MS Word and Excel preferred
• Excellent written and verbal communication skills
