Job Summary
To support a growing healthcare team, the full-time Integrated Kidney Care Advocate I will identify and validate hospital admissions for Transition of Care support and provide essential administrative assistance to care teams while working remotely.
Key responsibilities
- Confirming the status of existing admissions and conducting check-in calls to verify admission status
- Requesting and uploading medical records, completing manual mailing requests, and conducting provider verifications
- Providing administrative support by monitoring and processing incoming and outgoing faxes
Required qualifications
- High School Diploma required
- 2 years of clinical administrative experience preferred
- Intermediate proficiency in MS Word and Excel preferred
- Excellent written and verbal communication skills
