Job Summary

To support a growing healthcare team, the full-time Integrated Kidney Care Advocate I will identify and validate hospital admissions for Transition of Care support and provide essential administrative assistance to care teams while working remotely.

Key responsibilities
  • Confirming the status of existing admissions and conducting check-in calls to verify admission status
  • Requesting and uploading medical records, completing manual mailing requests, and conducting provider verifications
  • Providing administrative support by monitoring and processing incoming and outgoing faxes
Required qualifications
  • High School Diploma required
  • 2 years of clinical administrative experience preferred
  • Intermediate proficiency in MS Word and Excel preferred
  • Excellent written and verbal communication skills
