Iowa Licensed Case Management Nurse

Job Summary

To support members with chronic and complex health care needs, the remote-eligible Iowa Licensed Case Management Nurse will manage individualized care plans, coordinate services with health care providers, and engage members through various communication channels.

Key responsibilities

• Utilize critical thinking to identify high-risk conditions and proactively address member needs
• Design and manage individualized care plans while facilitating communication between members, families, and providers
• Document care management activities accurately and comply with regulatory standards and internal guidelines

Required qualifications

• Completion of an accredited nursing program
• Active and unrestricted RN License in Iowa or South Dakota
• 4+ years of diverse clinical experience in settings such as acute care or home health
• Must obtain Certified Case Manager (CCM) certification within 24 months of hire
• Strong technical acumen with experience in Microsoft Office products
