Iowa Licensed Case Management Nurse

Job Summary

Dedicated to supporting members' chronic and complex health care needs, the remote-eligible Iowa Licensed Case Management Nurse will create and manage individualized care plans, coordinate services with health care providers, and foster strong relationships with members and their families.

Key responsibilities

• Utilize critical thinking to assess and identify high-risk conditions, proactively anticipating member needs in navigating the health care system
• Design and manage individualized care plans while coordinating communication between members, providers, and community resources
• Document care management activities accurately and in compliance with regulatory standards, ensuring quality and consistency in care delivery

Required qualifications

• Completion of an accredited nursing program
• Active and unrestricted RN License in Iowa or South Dakota
• Certified Case Manager (CCM); must obtain within 24 months of hire
• 4+ years of diverse clinical experience in settings such as acute care, outpatient, or home health
• Strong technical acumen with proficiency in Microsoft Office and clinical documentation platforms
