Job Summary
To support medication-related transitions, the full-time Iowa Licensed Pharmacy Technician will facilitate accurate medication reconciliation for patients in both inpatient and outpatient settings, manage discharge planning, and collaborate with healthcare teams, with potential for remote work after training.
Key responsibilities
- Interview patients and families to gather comprehensive medication histories and input data into the electronic health record
- Assist with discharge planning, including processing test claims and coordinating with outpatient pharmacists for prescription delivery
- Identify and resolve medication-related problems, ensuring patient safety during transitions between healthcare settings
Required qualifications
- High School diploma or certified equivalency (GED)
- 1 year of experience as a Pharmacy Technician
- Licensed Pharmacy Technician in the state(s) of practice
- National Certification from the Pharmacy Technician Certification Board or National Healthcareer Association
- Basic knowledge of computer applications, including Word and Excel
