Iowa/South Dakota Licensed Case Manager
Job Summary
To support members with chronic and complex healthcare needs, the remote-eligible Iowa/South Dakota Licensed Case Manager will create and manage individualized care plans, coordinate services with healthcare providers, and actively engage with members and their families.
Key responsibilities
- Utilize critical thinking to identify high-risk conditions and anticipate member needs within the healthcare system
- Design and manage individualized care plans while coordinating with members, families, and healthcare providers
- Document care management activities accurately and comply with regulatory and accreditation standards
Required qualifications
- Completion of an accredited nursing program
- Active and unrestricted RN License in Iowa or South Dakota
- 4+ years of diverse clinical experience in settings such as acute care or home health
- Must obtain Certified Case Manager (CCM) certification within 24 months of hire
- Experience in care navigation or care coordination is strongly preferred
