Job Summary

A company is looking for a Journey Patient Specialist to manage day-to-day operations related to hospice services.

Key Responsibilities
  • Develop and process journey referrals while ensuring accurate documentation and workflow
  • Manage chart closure processes and provide necessary documentation
  • Assist team members with telephonic patient contact and ensure timely face-to-face visits
Required Qualifications and Education
  • High School diploma or GED
  • Previous experience as a Journey Counselor or in a hospice-related role
  • Knowledge of medical terminology
  • Proficient in Microsoft Office Suite, including Teams
  • Must maintain any required professional licenses or certifications
