Job Summary
A company is looking for a Journey Patient Specialist to manage day-to-day operations related to hospice services.
Key Responsibilities
- Develop and process journey referrals while ensuring accurate documentation and workflow
- Manage chart closure processes and provide necessary documentation
- Assist team members with telephonic patient contact and ensure timely face-to-face visits
Required Qualifications and Education
- High School diploma or GED
- Previous experience as a Journey Counselor or in a hospice-related role
- Knowledge of medical terminology
- Proficient in Microsoft Office Suite, including Teams
- Must maintain any required professional licenses or certifications
