Job Summary
A company is looking for a Remote Admissions Coordinator Long-Term Care & Rehabilitation (RN or LPN).
Key Responsibilities
- Coordinate and oversee admissions and transitions of care for members entering long-term care, skilled nursing facilities, or rehabilitation settings
- Review clinical documentation and assessments to support appropriate placement based on member needs and Kansas Medicaid criteria
- Serve as the primary liaison between the health plan, SNF admission teams, hospital discharge planners, and providers to ensure timely placements
Qualifications
- Active, unrestricted Kansas RN or LPN license (required)
- Minimum of 2-3 years of experience in home health, long-term care, rehabilitation, or intake/placement coordination
- Knowledge of Kansas Medicaid/KanCare systems, authorizations, and level-of-care guidelines
- Proficiency with EMRs and care coordination technology
- Ability to work independently in a remote, fast-paced environment
