Kansas Licensed Behavioral Health Manager

Job Summary

A company is looking for a Behavioral Health Case Manager.

Key Responsibilities

• Monitor and manage reports to identify gaps in care for Follow-Up After Hospitalization (FUH)
• Contact members to complete Behavioral Health HEDIS assessments
• Document assessments and assist members with scheduling follow-up appointments

Required Qualifications

• Bachelor's degree or equivalent experience with clinical certification
• Clinician license required: LPC, LSCSW, LMSW, or RN with Behavioral Health clinical certification
• 0-2 years of experience in behavioral health (e.g., crisis intervention, case management, therapy, intake/assessment)
• Experience with medical records/EHR systems
• Strong proficiency in Excel, Outlook, and other computer systems
