Job Summary
Facilitating virtual group support sessions for family caregivers, the part-time Kansas Licensed Care Coach will create a safe environment for sharing and support, while documenting key themes and concerns raised during sessions.
Key responsibilities
- Facilitate ongoing caregiver support groups, adhering to Trualta's group format and materials
- Document key themes or concerns from each session and prepare accordingly based on group anonymity
- Follow escalation protocols to address urgent caregiver needs or risks and maintain confidentiality of shared information
Required qualifications
- Licensed Non-Physician Practitioner (NPP) such as a clinical nurse specialist, physician assistant, or clinical social worker
- Experience working with patients and families managing chronic illness or aging-related care needs
- Comfortable facilitating group discussions in a virtual setting
- Personal caregiving experience preferred
- Reliable availability for the assigned weekly schedule
