Kansas Licensed Care Manager (RN)

Job Summary

To support a growing healthcare initiative, the full-time Kansas Licensed Care Manager (RN) will develop and facilitate personalized care plans for members with complex physical needs while working remotely.

Key responsibilities

• Evaluate member needs and barriers to care, recommending and facilitating plans for optimal health outcomes
• Develop ongoing care plans in collaboration with providers and community resources to address unmet needs
• Provide education and resource support to members and their families regarding healthcare options and services

Required qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in case management, maternal-child health, or care coordination
• Current RN - Registered Nurse State Licensure or Compact State Licensure
