Job Summary

To provide personalized health evaluations in the comfort of patients' homes, a Kansas Licensed In-Home Physician will engage with individuals full-time, conducting comprehensive assessments and connecting them to necessary care while working remotely and locally as needed.

Key responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical assessments
• Build trusted relationships with patients to improve health outcomes and facilitate connections to appropriate care
• Utilize an iPad and clinical workflow for evaluations, ensuring compliance with health plan requirements and conducting virtual visits as necessary

Required qualifications

• Active, unrestricted medical license in Kansas
• Preferred multi-state licenses for broader coverage
• MD or DO degree from an accredited medical school
• Experience in providing patient-centered care in diverse home environments
• Ability to travel locally and conduct virtual visits as required
