Job Summary

Engaging with patients in their homes, the part-time Kansas Licensed In-Home Physician will conduct health evaluations, including medication reviews and physical assessments, while utilizing an intuitive clinical workflow and ensuring personalized care delivery.

Key responsibilities

• Conduct in-home health evaluations lasting approximately 45 minutes, including medication and medical history reviews
• Build trusted relationships with patients to improve health outcomes and facilitate connections to appropriate care
• Perform virtual visits and provide diagnostic and preventive services as needed, while traveling locally within licensed states

Required qualifications

• Active, unrestricted medical license in the coverage area
• Preferred multi-state licenses
• Experience in conducting health evaluations in diverse home environments
• Ability to engage effectively with patients and their families
• Willingness to travel locally for patient visits
