Job Summary

To provide personalized health evaluations in patients' homes, the part-time Per Diem Kansas Licensed In-Home Physician will engage with individuals, conduct assessments, and bridge gaps in care while working remotely and locally as needed.

Key responsibilities

• Conduct in-home health evaluations, including medication reviews and physical assessments
• Build trusted relationships with patients to improve health outcomes and connect them to appropriate care
• Utilize technology to manage clinical workflows and perform virtual visits as necessary

Required qualifications

• Active, unrestricted medical license in Kansas
• Experience in providing in-home or community-based care
• Ability to travel locally and within licensed states as needed
• Comfortable working in diverse home environments with potential allergens
• Multi-state licenses preferred
