Job Summary

To support a growing network of in-home health evaluations, a Kansas Licensed In-Home Physician will engage with patients in their homes on a part-time basis, conducting health assessments and bridging gaps in care without prescribing medications or altering treatment regimens.

Key responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical assessments
• Build trusted relationships with patients to improve health outcomes and connect them to appropriate care
• Utilize technology for clinical workflows and perform virtual visits as needed

Required qualifications

• Active, unrestricted medical license in Kansas
• Preferred multi-state licenses
• Experience in providing patient-centered care in diverse home environments
• Ability to travel locally and within licensed states
• Commitment to improving health outcomes for patients
