Job Summary

Providing in-home health evaluations, the full-time Kansas Licensed In-Home Physician will engage with patients to assess their health needs and bridge gaps in care, utilizing an iPad for evaluations and conducting virtual visits as necessary.

Key responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical assessments
• Build trusted relationships with patients to improve health outcomes and connect them to appropriate care
• Travel locally within licensed states and perform virtual visits as needed

Required qualifications

• Active, unrestricted Kansas medical license
• Experience in providing in-home or community-based healthcare
• Ability to perform evaluations in diverse home environments
• Multi-state licenses preferred
• Commitment to improving patient health outcomes
