Job Summary

To provide personalized health evaluations in clients' homes, a Kansas Licensed In-Home Physician will engage with patients full-time, conducting comprehensive assessments and connecting them to necessary care while working remotely and locally as needed.

Key responsibilities

• Conduct in-home health evaluations, including medication reviews and physical assessments
• Build trusted relationships with patients to improve health outcomes and facilitate connections to care
• Perform virtual visits and provide diagnostic and preventive services as required

Required qualifications

• Active, unrestricted medical license in Kansas
• Multi-state licenses preferred
• MD or DO degree from an accredited medical school
• Experience in patient-centered care and home health evaluations
• Ability to work in diverse home environments
