Job Summary

To provide personalized health evaluations in the comfort of patients' homes, a Kansas Licensed In-Home Physician will engage with individuals on a per diem basis, conducting assessments and connecting them to necessary care while working part-time remotely.

Key responsibilities

• Conduct in-home health evaluations, including medication reviews and physical assessments
• Build trusted relationships with patients to improve health outcomes and facilitate connections to appropriate care
• Perform virtual visits as needed and provide diagnostic and preventive services as indicated

Required qualifications

• Active, unrestricted medical license in the coverage area
• Preferred multi-state medical licenses
• Experience in conducting in-home health evaluations
• Ability to work flexibly and travel locally as required
