Job Summary

Seeking a Kansas Licensed In-Home Physician for a part-time per diem position, responsible for conducting in-home health evaluations, building relationships with patients, and providing personalized care in a remote capacity.

Key responsibilities

• Conduct in-home health evaluations, including medication reviews and physical assessments
• Engage with patients to answer health questions and provide connections to appropriate care
• Utilize an iPad and clinical workflow to perform evaluations and document findings

Required qualifications

• Active, unrestricted medical license in the coverage area
• Multi-state licenses preferred
• Experience in providing patient-centered care in diverse environments
• Ability to travel locally and within licensed states as needed
• Willingness to conduct virtual visits when necessary
