Job Summary

A company is looking for a LPN Care Navigator to manage patient care and provide health coaching remotely.

Key Responsibilities

• Manage a panel of 400-450 patients, conducting monthly outreach and daily calls for care plan support
• Collaborate with clinical teams to address patient needs and support care coordination goals
• Document all patient interactions in real time while maintaining HIPAA compliance

Required Qualifications

• Active Licensed Practical Nurse or Licensed Vocational Nurse license with a multi-state compact license
• Minimum 2 years of clinical experience
• Experience working with EMR systems
• Familiarity with chronic condition management and individualized care planning
• Intermediate to advanced computer skills for navigating multiple systems
