Job Summary

A company is looking for a Nurse Advocate.

Key Responsibilities

• Educate, coach, monitor, and coordinate resources for individuals with identified health risks
• Provide telephonic and web-based outreach, health coaching, education, and navigation assistance
• Document client outreach, engagement, referrals, and follow-up while assisting in the development of health and wellness programs

Required Qualifications

• Current unencumbered Registered Nurse Licensure
• Minimum of three years nursing experience with chronic conditions and condition management
• Current BLS certification
• Competence in Microsoft Office and web-based programs
• Bilingual Spanish speaker strongly preferred
