Job Summary
Engaging with patients in their homes, the part-time Kansas Licensed Nurse Practitioner will conduct in-home health evaluations, focusing on medication reviews, physical assessments, and connecting individuals to appropriate care while utilizing an intuitive clinical workflow.
Key responsibilities
- Conduct in-home health evaluations lasting approximately 45 minutes to assess patients' health needs
- Build trusted relationships with patients to improve health outcomes and provide personalized care
- Perform virtual visits and provide diagnostic and preventive services as required
Required qualifications
- Active, unrestricted Nurse Practitioner license in the coverage area
- Board certification as a Nurse Practitioner
- Multi-state licenses preferred
