Job Summary
Engaging with patients in their homes, the part-time Kansas Licensed Nurse Practitioner will conduct in-home health evaluations, focusing on medication reviews, physical assessments, and connecting individuals to appropriate care while utilizing an iPad for clinical workflows.
Key responsibilities:
- Conduct in-home health evaluations, including medication and medical history reviews
- Build trusted relationships with patients to improve their health outcomes
- Perform virtual visits and provide diagnostic and preventive services as needed
Required qualifications:
- Active, unrestricted Kansas nursing license
- Board certification as a Nurse Practitioner
- Multi-state licenses preferred
