Job Summary

Seeking a Kansas Licensed Nurse Practitioner for a part-time per diem role, the clinician will provide in-home health evaluations, engaging with patients to bridge gaps in care while working remotely and traveling locally as needed.

Key responsibilities:

• Conduct in-home health evaluations, including medication and medical history reviews and physical assessments
• Build trusted relationships with patients to improve their health outcomes and connect them to appropriate care
• Perform virtual visits and provide diagnostic and preventive services as required

Required qualifications:

• Active, unrestricted license(s) in coverage area(s)
• Board certification (required)
• Multi-state licenses (preferred)
