Job Summary

Engaging with patients in their homes, the part-time Kansas Licensed Nurse Practitioner will conduct in-home health evaluations, including medication reviews and physical assessments, while utilizing an iPad and clinical workflow to enhance patient care.

Key responsibilities

• Conduct in-home health evaluations and provide personalized health assessments
• Build trusted relationships with patients to improve health outcomes and connect them to appropriate care
• Perform virtual visits as needed and travel locally within licensed states for patient evaluations

Required qualifications

• Active, unrestricted Kansas Nurse Practitioner license
• Board certification in relevant specialty
• Multi-state licenses preferred
