Job Summary

To support a growing network of in-home health evaluations, the part-time Per Diem Kansas Licensed Nurse Practitioner will engage with patients in their homes, conducting health assessments and bridging gaps in care while utilizing an intuitive clinical workflow.

Key responsibilities:

• Conduct in-home health evaluations, including medication and medical history reviews, and physical assessments
• Build trusted relationships with patients to improve health outcomes and connect them to appropriate care
• Flexibly travel locally and conduct virtual visits as needed, providing diagnostic and preventive services

Required qualifications:

• Active, unrestricted Kansas nursing license
• Board certification as a Nurse Practitioner
• Multi-state licenses preferred
