Job Summary

Providing in-home health evaluations, the part-time Kansas Licensed Nurse Practitioner will engage with patients to conduct assessments, review medical histories, and facilitate connections to appropriate care while working remotely and traveling locally as needed.

Key responsibilities:

• Conduct in-home health evaluations, including medication and medical history reviews and physical assessments
• Build trusted relationships with patients to improve health outcomes and provide personalized care
• Utilize an iPad and clinical workflow for evaluations, ensuring comprehensive service delivery

Required qualifications:

• Active, unrestricted Kansas nursing license
• Board certification as a Nurse Practitioner
• Multi-state licenses preferred
