Job Summary

Seeking a Kansas Licensed Nurse Practitioner for a part-time per diem position focused on conducting in-home health evaluations, engaging with patients to bridge gaps in care, and providing personalized health assessments remotely.

Key responsibilities

• Conduct in-home health evaluations, including medication reviews and physical assessments
• Build trusted relationships with patients to improve health outcomes and connect them to appropriate care
• Utilize technology for clinical workflows and perform virtual visits as needed

Required qualifications

• Active, unrestricted Kansas nursing license
• Board certification as a Nurse Practitioner
• Multi-state licenses preferred
• Ability to travel locally within licensed states
• Experience in providing diagnostic and preventive services is a plus
