Job Summary

To provide in-home health evaluations, the part-time Per Diem Kansas Licensed Nurse Practitioner will engage with patients in their homes, conducting assessments and connecting them to necessary care while using an intuitive clinical workflow.

Key responsibilities

• Conduct in-home health evaluations, including medication reviews and physical assessments
• Build trusted relationships with patients to improve health outcomes and provide personalized care
• Travel locally as needed and conduct virtual visits to accommodate patient needs

Required qualifications

• Active, unrestricted license(s) in coverage area(s)
• Board certification (required)
• Multi-state licenses (preferred)
