Job Summary

To provide personalized health evaluations in the comfort of patients' homes, the part-time Per Diem Kansas Licensed Nurse Practitioner will engage with individuals, conduct health assessments, and utilize an intuitive clinical workflow, while traveling locally as needed.

Key responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews, and physical assessments
• Build trusted relationships with patients to improve health outcomes and connect them with appropriate care
• Perform virtual visits and provide diagnostic and preventive services as indicated

Required qualifications

• Active, unrestricted Kansas Nurse Practitioner license
• Board certification as a Nurse Practitioner
• Multi-state licenses preferred
