Job Summary

Providing in-home health evaluations, the part-time Per Diem Kansas Licensed Nurse Practitioner will engage with patients in their residences, conducting assessments and connecting them to necessary health resources while utilizing an intuitive clinical workflow.

Key responsibilities

• Conduct in-home health evaluations, including medication reviews and physical assessments
• Build trusted relationships with patients to improve health outcomes and bridge gaps in care
• Perform virtual visits and provide diagnostic and preventive services as needed

Required qualifications

• Active, unrestricted license in the coverage area
• Board certification is required
• Multi-state licenses are preferred
