Job Summary

Seeking a Kansas Licensed Nurse Practitioner, the part-time clinician will provide in-home health evaluations, engaging with patients to review medical history and perform physical assessments, while working remotely and traveling locally as needed.

Key responsibilities:

• Conduct in-home health evaluations, including medication reviews and physical assessments
• Build trusted relationships with patients to improve health outcomes and connect them to appropriate care
• Utilize an iPad and clinical workflow to document evaluations and provide diagnostic services as indicated

Required qualifications:

• Active, unrestricted license in coverage area(s)
• Board certification
• Multi-state licenses preferred
