Job Summary

Collaborating with providers to manage patient care plans, the full-time Kansas Licensed Registered Nurse will perform remote nursing assessments, provide administrative support, and assist with virtual patient care while ensuring high-quality customer service in a clinical setting.

Key responsibilities

• Conduct remote nursing assessments and triage patient care needs for both virtual and in-person visits
• Assist providers with scheduling, referrals, and managing patient interactions in the EMR system
• Outreach to members to enhance engagement and support with social determinants of health

Required qualifications

• Completion of ASN or BSN with active RN licensure in Kansas
• Experience in patient triage and proficiency with EMR systems
• Ability to navigate web-based platforms for scheduling and reporting
• Flexibility to work independently and as part of a virtual team
• Knowledge of community resources related to health and social support
