Kentucky Licensed Addiction Medicine Provider

Job Summary

To support a growing practice, the fully remote Kentucky Licensed Addiction Medicine Provider will provide evaluations, treatment, and ongoing support for individuals with substance use disorders while collaborating with an interdisciplinary team.

Key responsibilities

• Provide initial evaluations, diagnoses, and ongoing support for patients with complex addiction issues
• Complete documentation to support professional billing and maintain patient care records
• Educate patients and families on treatment plans and medication compliance, while coordinating care with other healthcare providers

Required qualifications

• Active and unrestricted medical license as a physician or nurse practitioner in Kentucky
• Experience in providing addiction medicine services to complex populations
• Current DEA licensure and prescriptive authority as applicable
• Ability to work collaboratively with diverse care teams
• Proficiency in using Electronic Medical Records
