Kentucky Licensed Ambulatory Care Nurse

Job Summary

Providing clinical care management services, the full-time Kentucky Licensed Ambulatory Care Nurse will coordinate care for patients to improve health outcomes and manage chronic conditions, primarily working remotely with occasional onsite collaboration in the Paducah, KY area.

Key responsibilities

• Utilize Motivational Interviewing techniques to engage patients in self-management of chronic conditions
• Develop and implement care plans, performing medication reviews and ensuring patient understanding of their medical regimens
• Collaborate with healthcare providers to execute patient-centered care plans and manage referrals to community resources

Required qualifications

• Associate's Degree in Nursing (required)
• RN compact license or Multi-State License (required)
• 2-3 years of experience in acute care, home health, or case management
• Case Management certification (preferred)
• Bachelor's Degree in Nursing (BSN) (preferred)
