Kentucky Licensed Care Manager

Job Summary

A company is looking for a Care Manager Transition of Care.

Key Responsibilities

• Evaluate member needs through post-discharge assessments and medication reconciliation
• Develop care/service plans and collaborate with healthcare teams for member transitions
• Facilitate education and resources for members and coordinate services with providers

Required Qualifications, Training, and Education

• Master's degree in Behavioral Health or Social Work, or a Degree from an Accredited School of Nursing
• 2 - 4 years of related experience
• Licensure as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN required
• Must reside in Kentucky or Mississippi
• For RN, must have a compact license or be willing to obtain one
