Kentucky Licensed Care Manager

Job Summary

A company is looking for a Care Manager (RN) to develop and facilitate care management activities for members with physical needs.

Key Responsibilities

• Evaluate member needs and recommend care plans to address barriers to accessing appropriate care
• Collaborate with providers and community resources to develop and monitor ongoing care plans
• Provide education and support to members and their families regarding healthcare options and resources

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in case management preferred
• Kentucky RN or Compact RN license required
• Bilingual in English highly preferred
• Experience with telephonic care management is a plus
